How to Sign Up to be a Medicaid Provider |

Please help us in making the Medicaid Access-to-Care Task Force's vision a reality by enrolling in the
Medicaid program today. To sign up to become a Medicaid provider, visit www.lamedicaid.com and click on the
"Provider Enrollment" link to download the "Application for New
Enrollments" or call (800) 473-2783 for further information or assistance.
Once you have enrolled, please notify the LDA of your participation by filling
out the following form.
Your participation is vitally important to Louisiana's children.
