Screenings are conducted every Monday and Tuesday from 4 p.m. to 7 p.m. at Family Road in Baton Rouge , which is located at 323 East Airport Ave. The second screening location is held the 1^st and 3^rd Tuesday of each month from 4 p.m. – 6 p.m. in Gonzales at the First Baptist Church , which is located at 1217 South Bernside . No appointments are needed at either screening location.

To qualify for medical and dental services:

• Applicant must be currently employed and work a minimum 30 hours each week.
• Need to have worked the last 10 out of 12 months.
• Cannot have any medical or dental insurance of any kind, or receive any kind of assistances such as Medicaid.
• Must meet an income guideline that will discussed at the first screening meeting.

To receive treatment, applicant must bring:

• Most recent federal income tax statement
• The last two most recent paycheck stubs
• Photo ID
• Proof of residency such as utility bill

The Pediatric Dentistry Department of the LSU School of Dentistry is currently accepting new pediatric dental patients.  There are clinics in both New Orleans and Baton Rouge.

To qualify, a family must meet 350% of the federal poverty guidelines (Example: A household of four with an income of $70,000/year would still qualify.) We do require that applicants who qualify for LaChip receiving treatment complete a LaChip application within 90 days of treatment. We visit nine specific locations by appointment only. We see emergencies the same day.
